Milia vs comedone: which one are the tiny white bumps on your face, and will squeezing them make things worse?

Read this before you reach for a needle, a harsher scrub or another acne cream.

Hello, I'm Sarang Lim, M.D., Head Doctor.

You'll learn how to tell the two apart in a mirror, which removal methods suit each, what decides the cost, and how much recovery to plan around a short trip to Seoul. That makes it easier to pick one approach that fits your skin and your budget.

Hello, I'm Dr. Sarang Lim, Head Doctor at Muse Clinic Myeongdong in Seoul.

We're a one-minute walk from Myeongdong Station Exit 5 and open on weekends, Saturday and Sunday from 9:00 AM to 3:00 PM. English, Japanese and Chinese interpreters are in the clinic, and someone stays with you from consultation through treatment. Foreign patients see the same price list as Korean locals, with no foreigner markup.

I'm writing this because the two bumps look almost identical in a bathroom mirror, and treating milia like acne, or acne like milia, costs weeks and irritates skin that didn't need it.

A milium is a tiny keratin cyst sealed under the skin with no opening, so acne products rarely shift it and it usually has to be opened and lifted out. A closed comedone is a plugged pore inside a hair follicle, so it responds to pore-clearing skincare and acne treatment over weeks.

Not sure which one you have? Take one close-up photo of the bumps in daylight, without makeup, and send it on WhatsApp with the one question on your mind, such as whether a weekend visit is enough.

Milia vs comedone: how do you tell them apart in a mirror?

Start with color and feel. Milia are pearly white or slightly yellow, firm like a grain of sand, and sit right under the surface. Closed comedones are usually skin-colored or faintly white, softer, and often easier to see when you stretch the skin. DermNet, the New Zealand dermatology reference, describes milia as harmless keratin-filled cysts most often found on the eyelids and cheeks, while closed comedones turn up most often on the forehead and chin.

BehaviorStays the same for monthsComes and goes, can turn red

The behavior row tells you the most. A comedone is part of acne, so it can become a red pimple, and you usually have a mix of sizes at once. A milium rarely gets inflamed. It just sits there.

A few other bumps get mistaken for both. Syringomas are small skin-colored bumps under the eyes that come from sweat-duct tissue and have nothing to squeeze out. Flat warts are slightly rough, can spread in lines after shaving or scratching, and are caused by a virus, which is why anyone searching milia vs warts should stop picking until it's been looked at. Sebaceous hyperplasia shows up as soft yellowish bumps with a tiny central dip, usually from the 30s onward. If what you see doesn't fit either column of the table, have it examined before anyone tries to remove it.

Why does squeezing work on one and not the other?

A closed comedone has a pore. It's a follicle plugged with keratin and oil, and the plug has somewhere to go once the opening is loosened. A milium has no pore at all. It's a little ball of keratin trapped in its own sealed pocket, so pressing on it pushes against skin from every side.

That's why squeezing milia at home tends to leave a red mark next to the same white dot. Squeezing comedones has its own problem: DermNet lists follicle rupture from squeezing as one of the things that keeps comedonal acne going. In both cases, the fingernail is the wrong tool.

It also explains why acne creams disappoint people with milia. Products that loosen plugged pores, such as salicylic acid or retinoids, act on the follicle. They can help widespread milia slowly, but a single firm milium under the eye usually needs a tiny opening made with a sterile needle or blade.

Which removal methods fit each bump?

For milia, the standard medical options are few. The most common is de-roofing: a fine sterile needle or blade makes a small opening and the keratin is lifted out. For stubborn or numerous milia, doctors may use cautery or freezing, and for very extensive ones, peels or laser resurfacing. Topical retinoids help when there are many. DermNet also notes that many milia clear on their own within a few months.

For closed comedones, most of the work happens at home. DermNet lists comedolytic products applied as a thin layer once or twice a day, and is clear that improvement takes weeks to months. The American Academy of Dermatology's acne guidance likewise lists retinoids among the treatments dermatologists use for whiteheads and blackheads, with procedures added when skincare alone isn't enough.

Here is how that maps onto our acne menu in Myeongdong:

  • A prescription for acne is given after a consultation with me, for comedones that need more than drugstore products.
  • Capri Laser is aimed at the sebaceous glands behind breakouts, which makes it a conversation for comedones that keep coming back, and not a tool for milia.
  • Venus Viva uses needle radiofrequency to improve pores, skin texture and scarring, so it belongs later, if years of comedones have left rough texture behind.

If pitted marks are what's left, the session and cost questions change, and I go through them in our guide to acne scar treatment cost in Seoul. For milia right around the eyes, our post on getting rid of milia under the eyes covers that delicate area in more detail.

When a bump is perfectly round, pearly white and hasn't changed in months, I treat it as a milium until the exam says otherwise, and I don't reach for an acne laser for it. Matching the method to the bump, with genuine products at the correct dose, matters more to me than doing more. A structured consultation with an interpreter comes first, and then I plan and perform the treatment one-to-one.

An example of how I'd reason

Say you're 26, teaching English in Daejeon, with eight hard white dots under your eyes and a band of small rough bumps across your forehead that gets worse before your period. Those are probably two different problems. The dots under the eyes look like milia, which can usually be opened and lifted out in one visit. The forehead looks like closed comedones, which won't disappear in a weekend. For those, the plan that fits is a routine you keep using at home, with an in-clinic option only if they keep returning after a few months.

Milia removal recovery and what a short trip allows

After de-roofing, each spot usually leaves a pinpoint red dot or a tiny scab that settles within a few days to about a week. Many people go back to makeup once the openings have closed, often after a day or two, but around the eyes I'd keep products off the spots until they've sealed. Cautery or laser leaves a slightly bigger crust that usually takes around a week to fall off.

Comedone treatment runs on a different clock. A prescription routine often makes skin dry or flaky for the first few weeks, and the change shows over a couple of months. That's why a Seoul trip is the time to start the plan, not to finish it.

Keep the day of your flight out of it if you can, so any tiny scabs have a night to settle before you sit in dry cabin air.

If your trip dates are already set, send them on WhatsApp with a note on where the bumps are, under the eyes, forehead or chin, and ask what fits into those days.

What decides the cost of milia removal or comedone treatment?

I won't put a figure here, because the number comes from your face, not from a menu line. These are the things that move it:

  1. 01Which bump you actually have. Opening milia is a different procedure from treating comedones, and many people have both.
  2. 02How many and where. Three milia on a cheek are a short job; a cluster along the lower eyelid takes more time and care.
  3. 03The method. De-roofing, cautery, a prescription routine or a laser session each sit on a different line of a price list.
  4. 04Sessions. Milia are often a one-visit job. Comedones that need an in-clinic option may need repeat sessions spaced weeks apart.
  5. 05What continues at home. A routine you keep using adds to the total over months, even when the clinic visit itself is short.

Foreign patients see the same price list as Korean locals, and every treatment's price is published on our site. To get the figure for your own case, ask on WhatsApp what it would cost, with a photo of the area.

Are forum reviews right about milia and comedones?

Threads on milia vs closed comedones repeat a handful of claims. Some hold up and some don't.

“Exfoliate harder and they'll go away.” Mostly wrong. Gentle chemical exfoliants can help comedones over weeks, but hard scrubbing irritates skin, and DermNet lists abrasive washing as a trigger for comedones. It does very little for a sealed milium.

“Milia come from heavy eye cream.” Sometimes true. Rich, occlusive products around the eyes are commonly blamed, and DermNet notes that certain topical medicines, including steroid creams, are linked to milia. If new milia keep appearing where you apply one product, stopping it is worth trying.

“You can pop milia at home with a needle.” Risky. The opening has to be shallow and clean, and on the eyelid a slip is millimeters from the eye. Infection and marks are more likely with a household needle than a sterile one.

Many clear within a few months without treatment; the ones that have already lasted a long time are the ones that tend to stay.

Risks, and when to leave them alone

Removing milia or treating comedones is minor work, and minor work still has side effects.

Risks and side effects to know

  • Common and short-lived: redness, pinpoint scabs and mild tenderness, usually settling within days to a week.
  • Less common: a milium that returns in the same spot, or new milia forming after skin trauma, which DermNet notes can follow burns or dermabrasion.
  • Marks: on medium and darker skin tones, any inflammation can leave temporary brown marks. DermNet's page on post-inflammatory hyperpigmentation explains why these can take months to fade and why sun protection matters meanwhile.
  • Rare: infection or a small scar, more likely after squeezing or non-sterile tools.

Who should wait or skip it: anyone with an active skin infection or cold sore near the area, a laser or peel on the same spot in the past few weeks, current or recent oral isotretinoin, or pregnancy or breastfeeding when a retinoid is being considered. These are general medical cautions, and whether you're a candidate is decided at the in-person exam.

If a spot gets hot, swollen, increasingly painful or starts to ooze after you fly home, see a local doctor promptly and keep photos of how it changed day by day. Like any skin treatment, results vary from person to person, and so does how quickly skin settles.

If the milium you want gone sits right on the edge of the eyelid, I'd rather leave a harmless bump alone than take a risk that close to the eye. Milia are cosmetic. Nothing about them makes it worth rushing.

Finding us on a weekend in Myeongdong

We're on the 3rd floor of Korea Daeyungak Tower, 97 Toegye-ro, Jung-gu, a one-minute walk from Myeongdong Station Exit 5 on Line 4 and across from Shinsegae Department Store. The building has a Twosome Place café and Hana Bank on the first floor, which makes it easy to spot from the street.

Weekday hours are 10:00 AM to 8:00 PM, and Saturday and Sunday 9:00 AM to 3:00 PM, so a Saturday morning visit still leaves the afternoon for the city. You can read about my training on the doctor page and browse the full treatment menu before you come.

Your first WhatsApp message can be short: the bumps in one daylight photo and the question you care about most. You can ask before booking anything. When you come in, I'll examine the bumps in person and recommend the method that fits them, and we'll go through the details together at that consultation.

Questions people ask

Before you message us

Can milia turn into acne?

Usually not. A milium is a sealed keratin cyst with no pore, so it rarely becomes inflamed the way a comedone can. What does happen is that people have both at once, often milia around the eyes and closed comedones on the forehead or chin. Treating them as one problem is the common mistake, because the methods that clear comedones do little for a single firm milium.

Can I wear makeup after milia removal?

Many people can go back to makeup once the tiny openings have closed, often after a day or two. Around the eyes it's sensible to keep products off the treated spots until they've sealed, and to use a clean brush or fingertip when you start again. If a spot looks red, wet or crusted, give it more time before covering it.

Is it safe to remove milia under the eyes?

Milia under the eyes are commonly removed by de-roofing with a sterile needle or blade, which is a small procedure. The skin there is thin and the eye is close, so this is the area where home extraction causes the most trouble. Bumps right on the eyelid margin may be better left alone, since milia are harmless and many clear on their own.

Do closed comedones go away without treatment?

Some do, especially when a trigger such as a heavy cosmetic is removed. Many keep forming because the underlying tendency to plug pores is still there. Comedolytic skincare used consistently usually takes weeks to months to show a change, which is why comedones are better treated as an ongoing plan than a single appointment.

Does milia removal hurt?

Most people describe de-roofing as a series of quick sharp pricks rather than lasting pain, and each spot takes moments. Areas around the eyes can feel more sensitive. Some clinics use numbing cream for larger numbers of spots, so it's reasonable to ask about it when you book if you're nervous about needles near your face.

Results and maintenance period vary by individual. Every procedure carries the possibility of side effects, which are explained before you agree to treatment.
