Botox for Jowls: Why It Rarely Lifts, What Does

Botox for jowls is one of the most common searches that brings people to my consultation room, and one of the ones I most often have to redirect. Jowls are the soft pouches either side of the chin that blur the jawline. Botox relaxes muscle. Jowls are mostly loose skin and a fat pad that has slid down. Relaxing a muscle does not lift a pouch of skin and fat.

This guide explains why jowls form, when Botox has a real but small role, and what actually moves them on our menu at Muse Clinic Myeongdong: Thread Lifting, Ulthera Lifting, ALL TITE, and the NEW ACU Injection for fat under the jaw. Why jowls form: skin, fat and sometimes muscle

A jowl is not one thing. It is where several changes that come with age meet, and most people have more than one. The mix decides the treatment, which is why I look at the jaw before I name a device.

• Skin laxity: the dermis loses collagen and the skin along the jaw stops springing back, so it hangs below the bone line.
• Volume descent: the cheek fat pads lose support and slide down, gathering just in front of the jaw angle.
• A fat pocket: some people carry extra fat inside the jowl and under the chin, which no tightening will shrink.
• Masseter bulk: a large chewing muscle widens the lower face and makes a small jowl look heavier than it is.

Skin laxity and volume descent are the usual pair. Masseter bulk is the only one of the four where Botox does anything, and even then it narrows rather than lifts.

Botox for jowls: when it helps and when it is the wrong search

Botulinum toxin works by relaxing the muscle it is injected into. That is useful when a muscle is pulling or bulging. It does nothing for skin that has lost its spring or fat that has moved. So Botox for jowls, as most people mean it, treats the one cause that is least often the problem.

Where it does help: clench your teeth and feel the back corner of your jaw. If a firm bulge rises under your fingers, the masseter is part of your lower-face width, and jaw Botox will slim it over four to eight weeks. The jowl looks lighter because the jaw behind it is narrower. The pouch itself has not moved. I wrote a full post on masseter Botox if that is your case.

What actually lifts jowls without surgery: four options on our list

These are the treatments I use for jowls, in the order I usually consider them. Each has a short note on when I would choose it. Results and how long they last vary from person to person.

Thread Lifting: reposition the sag

Thread Lifting is the non-surgical way to restore a sagging face line. Threads are placed under the skin after local anaesthesia; they hold the tissue in a higher position and activate collagen along their path, so the face line firms up over the following months. The list runs from a 20-line mono thread session to 4-line QTL threads and One-Day packages with 8 lifting lines.

When I would choose it: a clear pouch you can lift with a fingertip and that stays lifted while you hold it. That is a mechanical problem and threads answer it mechanically. Expect bruising and swelling for one to two weeks, and plan a DuoDerm dressing on the needle spots for a day or two.

Ulthera Lifting: focused ultrasound for the deep layer

Ulthera Lifting uses genuine Ultherapy tips, FDA-cleared in the United States, and you can check the product originality before we start. Focused ultrasound heats the layer under the skin so it contracts and rebuilds collagen. The session takes less than 30 minutes and ends with a modelling mask and sunblock.

When I would choose it: skin laxity along the whole jaw and neck rather than one pouch, or a visitor who wants a natural, gradual lift with no needle marks to hide.

ALL TITE: radiofrequency tightening you feel the same day

ALL TITE is a radiofrequency lifting device built around DLTD technology. It delivers heat precisely from the dermis to the SMAS layer, so tightening can be felt straight after the session while collagen renewal continues for two to three months underneath. Little discomfort, back to daily life right after, and a recommended interval of once every three to six months.

When I would choose it: early jowls where the skin has just started to lose firmness, someone who cannot afford downtime, or a visitor who wants both an immediate result and collagen renewal from one visit. It works on sagging and wrinkles at the same time, which suits a jaw that has both.

NEW ACU Injection: dissolve the fat inside the pouch

The NEW ACU Injection is a concentrated, low-dose contour injection for the jawline, double chin and cheek fat. After an ice roller and a check of the product's originality and dose, the injection takes about 10 minutes and has no downtime on our list. It comes as Gold Accu or Premium Accu in 3cc and 10cc volumes.

When I would choose it: a jowl that feels soft and full rather than loose, or a double chin that joins the jowl in one curve. Fat has to be reduced before tightening shows, so I often do this first and add ALL TITE or Ulthera on top. On its own it slims, it does not lift.

Jowls treatment prices in Seoul: same list as Korean patients

Every number below is the line a Korean patient sees. There is no foreigner menu. Prices exclude 10% VAT, and event prices run for a limited period, so the treatment pages carry the current figures. Jaw Botox is included for the cases where masseter bulk is part of the picture.

What changes the number is shots and lines: 100 versus 300 shots of Ulthera, 400 versus 800 shots of ALL TITE, 4 versus 8 lifting threads, 3cc versus 10cc of contour injection. The larger Ulthera and ALL TITE sessions also let you add one option, such as tightening injections. I tell you the line I would pick for your jaw before you decide, not after.

The visit: consultation, treatment, aftercare

A structured consultation with an interpreter comes first. English, Japanese and Chinese interpreters are in the clinic, and someone stays with you from consultation through treatment. Then the plan is made and performed one to one by Dr. Sarang Lim, the head doctor.

1. 01Consultation: I pinch the skin along the jaw, press the pouch, and ask you to clench. That tells me skin, fat or muscle.
2. 02Face wash, then numbing: local anaesthetic for threads, an ice roller for injections.
3. 03Product check in front of you: Ultherapy tips and injectables are shown for originality and dose before use.
4. 04Treatment: under 30 minutes for Ulthera, about 10 minutes for the contour injection, longer for threads.
5. 05Finishing: mask and sunblock after Ulthera, soothing care after ALL TITE, DuoDerm on thread entry points.

Aftercare depends on what you had. After ALL TITE, washing and light makeup are fine the same day, but skip saunas, steam rooms, heavy drinking and smoking for a week. After threads, wash and apply makeup after two to three hours and expect the bruising to fade over one to two weeks. Side effects are possible with every procedure; they are discussed before you agree.
